Go to Applications> Utilities> Audio MIDI Setup and post a screengrab as to what you see. Also post what System Profiler has to say about your audio devices.

This keeps happening to me too. It happens at random, and I have no idea what's causing it.

Have you rebooted? That is really odd, unless some how you booted into safemode.
